Key dates for 2027 coverage
Medicare’s fall Annual Enrollment Period (AEP), also called Open Enrollment, runs from October 15 to December 7. During this window you can review Medicare Advantage and Part D coverage and make eligible changes for January 1. See Medicare’s enrollment rules.
- Before enrollment: Gather your renewal notice, medication list, and provider names.
- October 15, 2026: The annual enrollment window opens.
- December 7, 2026: Your plan must receive your enrollment request by this date.
- January 1, 2027: Coverage changes made during this window take effect.
“2027 coverage” and “AEP in 2027” mean different things. The fall 2027 enrollment window is for coverage beginning in 2028.
Confirmed Part D changes for 2027
CMS has published these national parameters. Costs and availability vary by plan and location.

The spending threshold applies to covered Part D drugs; premiums do not count toward it. The base beneficiary premium is used in Medicare’s calculations and is not a quote for your plan. Some plans have a lower deductible.
CMS also confirmed that the temporary premium stabilization demonstration for standalone drug plans ends after 2026. Check your own renewal materials rather than assuming a particular premium increase.

Your Inland Empire plan-review checklist
For residents of Ontario, Upland, Rancho Cucamonga, Chino, Fontana and neighboring communities, start with your ZIP code and the care you actually use. Availability can differ between San Bernardino and Riverside counties.
- Read your Annual Notice of Change. Highlight changes in premiums, copays, benefits and drug coverage.
- List your providers. Include your primary doctor, specialists, medical group and preferred hospitals. Confirm participation with the specific plan and provider.
- List prescriptions and pharmacies. Include medication names, doses and how often you fill them. Compare estimated yearly costs, not just the monthly premium.
- Review your full budget. Consider deductibles, specialist visits and hospital costs alongside additional benefits.
- Confirm before changing. Check the plan’s 2027 documents and effective date. Keep your enrollment confirmation.
If your insurer says your plan is ending, bring that notice to your review. Additional enrollment rights may apply; the notice and your circumstances determine your next steps.
If you have both Medicare and Medi-Cal, include your Medi-Cal plan information so coordination of benefits can be considered. For Nevada and Arizona appointments, we use your local plan area and providers.

When do I enroll for 2027 Medicare coverage?
The annual enrollment window is October 15 through December 7, 2026, for coverage starting January 1, 2027. Enrollment in fall 2027 is for the 2028 coverage year.
Is Annual Enrollment the time to first sign up for Medicare?
Annual Enrollment is for reviewing and changing Medicare health and drug coverage. If you are new to Medicare, your initial enrollment dates depend on your circumstances. Ask about your own enrollment window before waiting until fall.
Will my drug plan premium increase?
Your premium depends on your specific plan. CMS has confirmed that the temporary standalone Part D premium stabilization demonstration ends after 2026. Read your plan’s renewal materials to see your actual 2027 premium.
Can I switch to a Medicare Supplement during AEP?
Annual Enrollment does not automatically give you a right to buy a Medigap policy without medical underwriting. Check your eligibility and any applicable protections before dropping existing coverage.
